The hamstring muscles, located at the back of the thigh, are comprised of three muscle groups. Injuries to this muscle are common in athletes participating in sports such as football or basketball. This muscle group allows your knee to bend, and plays a critical role in every activity involving the lower body. Your hamstrings are contracting every single time you take a step, climb a flight of stairs or get in and out of your car.

The anatomy of this muscle group

The three muscle groups that comprise the hamstrings include the biceps femoris, semi-membranosus and semi-tendinosus. A partial or complete tear of either of these muscles is a likely possibility in hamstring injuries.

Cause of hamstring muscle injury

A sudden, excessive stress on this muscle group is a common cause of injury. An individual with a previous history of hamstring injuries is more likely to get re-injured. Hamstring muscle tightness and inadequate warm-up can also cause injuries.

It’s common for the physician’s office to recommend a provider if the office is familiar with or has an affiliation with that provider. The choice is yours. Did you know?

  • Most states allow the patient to see a physical therapist without a physician’s referral. If you don’t know whether you need a referral, don’t worry. Just call our office and we’ll get the answer for you.
  • Some insurance policies may require a visit to your primary care physician before attending physical therapy or may require you to use ‘preferred providers’ only. This is also something we can look into for you when you give us a call.
  • Your physician may advise you to visit their own in-office physical therapy facility or one that is run by a large hospital. You can do so, or research and use an independent physical therapist in private practice – the choice is yours.

Your first session with your physical therapist is called the initial evaluation. Your physical therapist will spend time learning about your condition, and how your condition is affecting your life. Your therapist will then take specific measurements of the impairments that may be causing your problem and put together a patient-specific treatment plan.

1. Show Up Early- It is suggested by the front desk staff at OSTC, that new patients show up about 30 minutes early to their initial evaluation. This will give you time to complete all paperwork and discuss insurance details with our patient care coordinators. If you have already completed the paperwork, it is advisable to show up at least 15 min. early to tie up any loose ends before your sessions begin.

2. Wear Appropriate Clothing- Your first day may require a lot of movement, with that in mind, be sure to wear comfortable-loose fitting clothing that allows for unrestricted movement. Also, your clothing should allow for access to your affected body part. If you are being treated for a shoulder injury, wear a tank top; if you are being treated for a knee injury, shorts are always a good option. OSTC has locker rooms available for you to change clothes, and the front desk staff can issue you a locker to store your belongings.

Where Does Physical Therapy Stand on Foot Drop Treatment (Dec 2018)

Foot drop syndrome is a neuro-muscular disorder characterized by weak muscles in the front of the lower leg. This can result in partial or total loss of control over the foot. This affects the ability to lift the foot at the ankle.

Damage to the nerve fibers that allow the flexing of the ankle and toes can cause several problems. For example, the toes point towards the floor when the foot is lifted up from the ground. When patients attempt to walk, they tend to drag the foot along the ground. They further compensate by lifting the knee higher than usual.

Foot drop can often be the sign of a major underlying complication, rather than a ‘simple’ inability to raise the foot. It can be caused by a nerve injury, spinal or brain disorder or muscle disorder. Foot drop can affect one foot or both feet and can be temporary or permanent, depending on the cause.

Physical therapy plays an important role in the management of foot drop. It allows patients to experience better mobility, which leads to an improved quality of life.

Total hip replacement (THR) is a surgical procedure to replace a damaged hip joint with a new artificial/prosthetic implant. Physical therapy is essential before and after surgery in order to minimize complications. Therapy also reduces recovery time and restores hip joint function as quickly as possible.

Musculo-skeletal conditions that may require THR

An impaired hip joint can result in pain and motion restriction for daily activities like sitting, standing, walking and commuting. Some of the conditions that could potentially damage the hip joint include:

  • Trauma

  • Rheumatoid arthritis
  • Osteonecrosis

The Lowdown on Stress Fractures (Oct 2 2018)

A stress fracture, also known as a fatigue-induced fracture, consists of one or more small cracks in the bone. It is associated with a pattern of overuse, commonly seen in the lower extremity in athletes.

Causes

Repetitive motion is one the mechanisms that result in a stress fracture. The muscles become fatigued and are unable to absorb ground force, which is transmitted to the bone. This overload results in stress fractures over a period. High impact sporting activities that result in this injury include:

  1. Track and field activities
  2. Basketball
  3. Tennis
  4. Gymnastics

Symptoms

The main symptom is pain in the extremities which worsens with activity and subsides with rest.

Diagnosis

As with any bone injury, the confirmatory diagnostic test is an x-ray. In some cases, a CAT scan or MRI is requested if the x-ray is not conclusive.

Treatment

The pain stimulus is the body’s natural mechanism to protect itself from severe self-inflicted injury. The rule of thumb for immediate treatment is:

  • Rest
  • Ice
  • Compression
  • Elevation

Rest (and limited weight bearing) is an important aspect of stress reduction on the bone and surrounding joints. Shoe inserts also help with shock absorption.

Getting your child involved in sports is an excellent way to encourage exercise and prevent childhood obesity. This is a proud feeling for every parent, and the expectation is that the child will have fun, gain confidence and interact with other children in the process.

To determine what your child will enjoy participating in, help him / her decide by accompanying the child to different games and a variety of sports. The more sports and environments the child is made aware of, the higher the probability that the child will be able to pick a sport and enjoy it. It is important to make sure that the child’s sporting activity is age appropriate.

In general, toddlers (aged 2-5) are too young to comprehend most organized activities and the importance of ‘rules’. They need unstructured play that allows them to develop movement skills, attention span and social maturity. From the age of 8 and above, children can participate in team sports and group exercise. If your child does not like athletic activities, take the opportunity to spend more time with your child and encourage physical activity with regular walks, swimming, tossing a ball around, or simply kicking a ball back and forth. Be creative and mix it up so your child does not get bored.

Knee and ankle injuries are common for people of all ages. They tend to occur during day-to-day activities such as walking, climbing stairs and running. However, the residual effects of knee and ankle injuries (loss of strength and mobility) are best treated with physical therapy.

The knee joint is a complex, weight-bearing structure and the ankle, in particular, is protected by ligaments on the inside, outside and the front. Sudden twisting movements can lead to tendon and ligament tears, and in some cases, fractures. Also, a myriad of injuries including strains, sprains, plantar fasciitis, fractures, meniscal tears and ligament tears can lead to impaired mobility and severe pain. These conditions can make it difficult, if not impossible for the individual to walk until the injury heals.

Depending on the severity of the injury, surgery may be required. With or without surgery, physical therapy plays a vital role in recovery from knee and ankle injuries.

The bones of the hip and pelvic region act as protection for internal organs such as the bladder. Depending on the severity of the injury, a hip fracture can result in internal bleeding, difficulty urinating and abdominal pain.

Fractures in the pelvis and hip typically result from high-impact trauma. Participation in athletic programs, bicycling injuries and vehicle accidents can also cause fractures.

Low-impact injuries in the elderly are usually precipitated by osteoarthritis and osteoporosis. Falls are a major area of concern for the elderly. Causes include deterioration in balance, impaired vision and unintended obstacles like slippery floors, rugs and even pets. Fractures as a direct result of falls in the elderly can lead to complications such as cardiovascular disorders, pulmonary problems and infections.

Why do elderly patients fall?

Visual problems, muscle weakness and low blood pressure can cause a loss of balance, which leads to a fall. Other reasons include conditions such as Parkinson’s disease and other conditions that affect the nerves.

Occasionally, middle ear problems (vestibular problems) can occur and balance can be affected. The good news is, falls can be largely prevented with the right precautions.

As one gets older, muscles and joints tend to degenerate, resulting in aches and pains.

The fact is, you may need to use a cane / walker or other assistive devices at some point in your life. Age-related pathologies like osteoarthritis or sudden traumatic injuries can lead to difficulty with daily tasks.

Physical therapy is an important part of the rehabilitation process. Physical therapists advocate the use of assistive devices as part of their treatment especially when patients experience decrease in mobility and restricted function.

Assisted walking devices help patients perform daily activities in a pain free manner, and also improve mobility. Assisted walking devices include, but are not limited to, canes, walkers, and crutches. Physical therapists often prescribe such devices following a detailed assessment of the patient’s deficits and requirements.

Can Physical Therapy Help with Shin Splints? (April 2 2018)

The shinbone or the tibia is the front surface of the lower leg. This part of the body is under constant stress and strain when we walk, jump and run.

An overuse injury happens when a certain threshold of tolerance is crossed. This is a condition called shin splints. Intense pain occurs on the front, inside, outside and at times, the back of the lower leg. If left untreated, this pain can lead to stress fractures of the tibia. Shin splints occur due to damage and inflammation of the muscle tendons attached to the tibia. The pain can be rather unbearable, forcing individuals to take prolonged periods of rest to allow for the inflammation to heal. Physical therapy plays an important role in the treatment of shin splints.

Visit your physical therapist if:

  1. There is a lingering pain after exercising or physical activity. The pain does not subside after 3-4 days of resting and icing the joint or limb.
  2. There is a dull pain, which recurs with activity.
  3. Pain does not resolve with over the counter pain relief medication.
  4. The lower leg is swollen or shows signs of bruising after activity

Degenerative disc disease, despite the name, is not a disease but a deterioration of the discs of the spine. It occurs over a period of time, resulting in neck or back pain and other musculoskeletal and neurological symptoms. It is common in adults.

Types of degenerative disc disease

  1. Cervical: affects the neck and is referred to as cervical degenerative disc disease
  2. Lumbar: affects the lower back and is called lumbar degenerative disc disease

The cervical and lumbar regions of the spine are susceptible to damage due to increased movement capabilities. Constant motion over time results in the wear and tear of the discs.

Signs and Symptoms of DDD

Most patients report one or more of the following symptoms.

  • Pain is triggered by an activity
  • Pain flares up periodically and then settles to a low-grade pain/discomfort
  • Sitting or standing for long periods of time worsen the pain
  • Activities such as walking may alleviate the pain
  • Relief upon changing body position
  • Muscle spasms
  • Tingling sensations in the extremities

Any activity that triggers pain in the neck or lower back should be ceased immediately. If there is no relief with rest, consult with a physical therapist. Early intervention could save you from further damage to the disc, muscles and ligaments.

Causes of a Wrist Fracture  (Feb 19 2018)

Typically, wrist fractures occur due to a fall on an outstretched hand.  When the fracture occurs at the end of a bone in the forearm called the radius (at the cortico-cancellous junction), it is called a Colles’ Fracture. Dorsal displacement and dorsal angulation are common characteristics of such a fracture. Falling on an outstretched hand can be a consequence of either tripping or losing balance, and it is the body’s defense mechanism against falling flat on one’s face. While this sort of fracture is not prevalent in younger patients, it is commonly seen in older patients, patients with osteoporosis, and those with any other form of bone disease.

The treatment of a wrist fracture requires the application of a cast to facilitate compression and prevent motion in an effort to promote healing. In some cases, surgical correction may be required. Once pain and swelling is reduced, the objective of the healthcare team changes. The new priority is joint range of motion and restoration of muscle strength. This is where physical therapy plays an important role.

One of the most important strategies for relieving your pain is a customized exercise program. Your physical therapist will create a program that factors in your strengths, areas that need improvement, any chronic conditions, and your fitness level.

Physical therapy will help ease pain, allow you to move better, and promotes restorative sleep necessary for the body to repair and rejuvenate itself. The knee joint is a complex, weight-bearing structure and the ankle, in particular, is protected by ligaments on the inside, outside and the front. Sudden twisting movements can lead to tendon and ligament tears, and in some cases, fractures. Also, a myriad of injuries including strains, sprains, plantar fasciitis, fractures, meniscal tears and ligament tears can lead to impaired mobility and severe pain. These conditions can make it difficult, if not impossible, for the individual to walk until the injury heals.

Many of our patients are initially unaware of the Wellness/Personal Training programs available at OSTC.  These programs are open to the general public for a low cost membership fee.  As an added bonus, any person who recieves physical therapy at OSTC is eligible for a free month of membership.  We have found that this helps clients transition out of physical therapy and continue their exercises.  During this time, patients can use all the equipment they are accustom to as well as direct access to their physical therapist should any questions arise.

• Personal Training

The personal trainers at OSTC are the best in area!  The professional wellness staff consists of certified strength and conditioning specialists (CSCS), nutritional coaches certified through National Association of Sports Medicine and Precision Nutrition, as well as a licensed massage therapist.

We offer great option for those who need some additional motivation. We help anyone from athletes looking for that extra edge, to those who just want to be healthy.  Our clients can choose to work with a CSCS one on one, in small groups or join our large group fit camps or pool classes.

Ankle Sprains and Physical Therapy (Sept 18 2017)

[accordion header=”” text=” The ankle is the most common site of sprains in the human body. An ankle sprain can happen to anybody at a time when you least expect it. Normally, it occurs after a sudden unnatural twisting of the foot which can tear the ligaments of the ankle. An ankle injury affects every aspect of your daily life, most notably your ability to walk and drive. The symptoms and sign of ankle sprains may vary depending on the severity. There might be tenderness, bruising, swelling and stiffness. Walking can be painful if the ankle sprain is serious.

A mild ankle sprain may not require a doctor’s attention but if any of the following happen after you twisted your ankle, consult your doctor.

• Uncontrollable pain even after taking medications

• You are unable to walk or cannot walk without severe pain.

• The ankle is swollen beyond normal and is not improving within 5-7 days.

Ankle sprain can be prevented by:

The body has three muscles (collectively known as the hamstring) in the back of the thigh that can be injured, with the most common cause being participation in sports and athletic endeavors. The hamstring muscle allows people to extend the hip and bend the knee.  Once a hamstring muscle has been injured, it can be up to six times more likely to be injured again.

Hamstring muscles are typically injured when an individual:

• Rapidly accelerates or stops suddenly while running

• Changes direction quickly

• When jumping

• Sprinting, hurdling or kicking

• During heavy lifting

Individuals are at increased risk if they don’t warm up and stretch sufficiently before engaging in new, unfamiliar or strenuous activities. Muscle fatigue, a prior injury and weakness in the hamstring or glutes are all factors that contribute to an injury. The damage can range from mild and feel like a cramp. Some people don’t realize they’ve even sustained an injury until they rest or during the following day.

In a severe hamstring injury, the individual may feel a “popping” sensation or a sharp pain in the back of the thigh that extends into the buttock. The area may be swollen, bruised and tender to the touch and there may be difficulty sitting, lifting the leg or stretching it out fully. If the muscle is torn, surgery may be needed to repair the damage.

The feet bear the weight of the body upon them, but the heel of the foot receives little thought unless it begins to hurt. Pain can originate under the heel (plantar fasciitis) or the back of the heel in the Achilles tendon. Inflammation, swelling and pain are common complaints associated with the heel.

An injury to any part of the heel can be extremely painful, producing inflammation and swelling. Individuals may have difficulty flexing their foot up or down, standing on their toes, or even walking. A heel injury may also result in a tingling sensation indicating the need for immediate medical care that will often require rehabilitation with a physical therapist.

Everyone has an occasional pain in the heel after extensive walking, jogging or sports activities and it typically disappears within a day or so with rest. Heel pain that interferes in standing, walking, or lasts a week or more can be helped with physical therapy.

Plantar fasciitis is the most common cause of heel pain resulting from running and jogging, obesity, and excessive standing. Pain may be present when people take their first few steps in the morning or after sitting down for an extended period of sitting.
