The UIL has updated the paperwork for the pre-participation physical. You can find the new paperwork here.
OSTC will be hosting our annual physicals this year on May 16th. As a reminder, this is a free service that we provide for schools that contract their sports medicine care through OSTC. Each school has been given a specific time slot to help reduce the risk of infection.

- Christ Academy 8:20
- Burkburnett 8:40
- Henrietta 9:00
- Notre Dame 9:20
With everything going on this year, we are practicing social distancing, so things will operate a little different in order to maintain a healthy environment.
Make sure to remind your athletes that they must print off and bring the Physical Form with them. We will not be printing them the day of. The form can be found on the UIL website or by clicking here
When athletes arrive, they need to stay in their vehicles and drive under the awning to check in. If it is prior to your time slot, you will be asked to wait in your vehicle as we are only allowing one school in the building at a time.
We ask that all parents remain in their vehicles while athletes are inside to reduce the number of bodies within the building.
Inside the building, we will be taking the following precautions:
- Six feet of space between each athlete waiting in line
- All employees will be wearing masks
- We will utilize private rooms during the examinations to keep social distance
- Our regular cleaning procedures have been amplified and are completed on a more frequent basis
The safety of our employees, patients, and athletes is of utmost importance, so rest assured that we are doing everything we can to provide physicals in a safe and healthy environment. Please be aware that the process may take slightly longer than normal, so please be patient with us.
